What is the Discharge Medicines Service?

Support that continues after you leave the ward

Patients are often discharged from hospital before they’ve fully recovered. This means your medicines may have changed, and there can be a lot to take in when you leave hospital.

Whether you’re recovering at home or elsewhere, it’s important to follow your doctor’s advice. While you’re on the mend, you shouldn’t have to worry about new medicines or how to take them. That’s where our experienced team can help.
